SPORTS PHYSICAL TIME

Sports physicals will be done at Plainview High School and at Plainview Area Health System.
Dr. Brad Lockee and Jeanna Simons, PAC will do physicals at the school June 22, July 19 and August 9 from 1-5 pm, no appointment necessary.
If students cannot make it to the school on these days, appointments can be made at Plainview Area Health System.
Dr. Lockee, Jeana Simons, PAC and Martha McCabe, PAC will be available at Plainview Area Health System by appointment.
The cost will be $25.00 during June, July and August and will need to be paid before the physical form will be released to the school. The history part of the form needs to be filled out by the parent and signed by both the parent and student before the physical is done. Forms can be obtained from Plainview Area Health System, at the school or downloaded from the link below. Please attach a copy of their immunization record.

Please bring a urine specimen in a disposable container labeled with student's name. If you do not have a container, one can be picked up at Plainview Area Health System. Hemoglobins will not be drawn this year.
PAYMENT IS DUE AT TIME OF SERVICE AND NO INSURANCE WILL BE FILED.
