PHYSICAL AND VACCINATION REQUIREMENTS FOR ENTERING SCHOOL

According to Nebraska State School Law, all students entering kindergarten must have a physical examination by a physician, physician’s assistant, or certified nurse practitioner with vision screening and be current on shots. 

All students entering seventh grade must have a physical examination by a physician, physician’s assistant, or certified nurse practitioner and be current on shots.  The physical must be performed within six months before entering school. 

All out of state students transferring in must have a physical and vision check and be immunized appropriately according to the grade entered.

All of the above physicals must have the complete physical form returned to the school nurse.

All students participating in a sport activity throughout the year are required to have a physical and are only required to return the Clearance Form and the Student and Parent Consent Form ( the last 2 pages) filled out to the school office.

Plainview Clinic will be doing physicals during regular hours.
